FOR OUR PATIENTS WITH DENTAL INSURANCE COVERAGE:
Please familiarize yourself with your insurance coverage directly with your insurance provider. On your request and as a courtesy to our patients, our office will attempt to file insurance claims on the patient’s behalf.
The amount of dental insurance coverage you receive will depend solely on the type of plan purchased by you or your employer, not the fees of our practice.
- Regardless of the status of an insurance claim, full payment is due to our office as at the date of service and is overdue after thirty (30) days from the date of service.
- Although an effort is made to determine your benefits, you are responsible for knowing the specific benefits of your plan, including the frequency and age limitations of your particular plan. For questions regarding the specifics of your plan, please contact your insurance company or human resource department. We can try assist you but in no way provide assurance on your insurance coverages.
- We do not guarantee what your insurance will or will not do with each claim. We are not be responsible for any errors in filing your insurance. Once again, claims are filed as a courtesy to you.
- We are not responsible for how your insurance company handles its claims, or for what benefits they pay on a claim.
- On your behalf we will only file your claim once. It is important that you provide us with accurate information on your insurance. Inaccurate insurance information and/or denial of coverage based on the insurance information that you provide will result in you being responsible for the entire charges.
Insurance plans do not cover the full cost of dental services and the patient is responsible for their co-pay portion. We require insurance co-pays for professional services rendered at the time of treatment, as well as any deductibles that have not been met.
Please remember that insurance is considered a method of assisting in the cost of care and is not a guarantee of payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge.
